Subject: Remove the HTML title= attributes which harmed accessibility and
 violated the principle of separation of content and presentation. Instead,
 implement the tooltips purely in CSS. Thanks to John Gardner <gardnerjohng at
 gmail dot com> for suggesting most of the styling in the new ::before rules.
